Personal conversation, not clipboards.

Most information about you as a patient is gathered through direct conversation — first with your surgeon at their office, and later with Dr. Rand Scott personally during your free Rest-Assure pre-operative evaluation for anesthesia planning. Both conversations are more informative than any form.

Some written documentation is still required by law (HIPAA, informed consent) and by AAAHC accreditation. Because NCSS is an affiliated-surgeon facility rather than a physician group, the paperwork you complete typically comes from two sources: your surgeon's office and NCSS. This page explains how that works.

The two-office paperwork model.

Your affiliated surgeon runs their own practice with their own office. NCSS is the surgical facility they choose to operate in. Paperwork reflects that structure.

• From your surgeon's office — the primary paperwork. Your surgeon's office typically provides the majority of your forms during your surgical pre-operative visit with them (usually 1–4 weeks before your procedure). This includes: surgeon's new-patient registration, detailed health history, procedure-specific informed consent, photography consent (photography is handled by your surgeon, not by NCSS — see 'Privacy by design' below), and the surgeon's financial responsibility acknowledgment. You complete these at home and bring the completed forms with you on surgery day.
• From NCSS — the facility-specific paperwork. Some documentation is specific to NCSS as the facility of record. These are minimal, delivered as direct PDF downloads by email or provided at your appointment: HIPAA acknowledgment (our practice's privacy notice), anesthesia consent, and facility-specific consents where applicable.
• Why there's some overlap. You may notice that your surgeon's health history and our brief pre-surgical verification cover similar ground. This is deliberate. We verify critical clinical information independently rather than assume it was captured correctly upstream. It's a safety practice, not a paperwork problem.

Two deliberate choices.

Some of what makes NCSS different is what we don't do. Two deliberate choices about paperwork and information handling:

• NCSS does not photograph patients as a surgical facility. Before-and-after photography, clinical documentation photos, and any marketing imagery involving patients are handled by your surgeon's office directly, not by us. This is a deliberate privacy practice at the facility level. Whether you consent to photography by your surgeon — and to what use it's put — is a conversation you have with your surgeon and their team.
• NCSS does not use a patient portal. Patient portal systems can create privacy exposure through account credentials, third-party integrations, and data-at-rest concerns that we have chosen not to accept. Our NCSS-specific forms are provided as direct PDF downloads by email, or in person at your appointment — not through an online patient account you must create and manage.
• Your surgeon may still operate their own portal. Some of our affiliated surgeons may operate patient portal systems for their own practice. That's separate from NCSS. Ask your surgeon's office about their specific privacy practices for their portal, if applicable.

When each piece happens.

Paperwork is spread across the pre-operative period so nothing is rushed on surgery day.

1. 1At your surgical consultation — conversation, minimal paperwork. Your first visit with your surgeon at their office is primarily evaluation and discussion. Any initial forms your surgeon's office uses (basic registration, brief health history) may be completed here.
2. 2At your surgical pre-op with your surgeon — the main paperwork visit. Once you have decided to proceed with surgery, your surgeon's office provides the bulk of your paperwork. Health history, procedure-specific informed consent, photography consent, financial responsibility. You take these home and complete them at your own pace before surgery day.
3. 31–2 weeks before surgery — Rest-Assure pre-op with Dr. Scott. Your free personal consultation with our medical director. This is focused on anesthesia planning, medication review, and discharge coordination — not administrative paperwork. Any final HIPAA or facility-specific acknowledgments related to anesthesia can be handled at this visit.
4. 4Day of surgery — final confirmations only. Arrival at NCSS. Your surgeon's completed forms are reviewed. Final acknowledgments and any last-minute updates. By design, you are not asked to complete substantial paperwork the day of surgery.
5. 5Post-op — discharge instructions. Written discharge instructions are provided at the end of your surgery day. Your support person receives verbal instructions in addition to the written ones.

The two-number rule.

Because paperwork comes from two offices, questions go to two offices. The rule is simple:

• For your surgeon's forms — call your surgeon's office. Health history questions, informed consent questions, photography consent, your surgeon's financial paperwork, requesting a replacement form. Your surgeon's office phone number is on the forms themselves and on their practice's website.
• For NCSS-specific forms — call us. HIPAA acknowledgment, anesthesia consent, facility questions. Call (949) 644-8182, Monday through Thursday, 8am to 3pm.
• Not sure which? Call either office and we'll direct you. Most surgeon's office staff know exactly what NCSS handles and vice versa.
• Language preference. Some of our affiliated surgeons and staff are fluent in Spanish and/or Italian. Free interpretation services are also available on request. Please let us know your preference.

The morning-of checklist.

On surgery day, please bring:

• Photo ID — driver's license or other government-issued ID
• Insurance card (if applicable) — even for self-pay procedures, we may keep this on file
• All completed forms from your surgeon's office — the packet they provided at your pre-op visit
• Complete list of current medications including dosages, or your medication bottles
• List of allergies including drug allergies and reactions
• Payment method for any remaining balance
